Abstract

Clinical pharmacy has transformed into a patient centred and evidence-based discipline that emphasizes the optimization of pharmacotherapy and the improvement of healthcare outcomes. The integration of emerging technologies such as artificial intelligence, pharmacogenomics, and digital health platforms has further strengthened clinical decision-making and patient monitoring. Patient care analysis plays a central role in clinical pharmacy by enabling systematic evaluation of therapeutic regimens, identification of drug-related problems, and development of individualized treatment strategies. This article presents a comprehensive and updated theoretical overview of clinical pharmacy practice, including its conceptual foundations, evolving roles, structured approaches to patient care analysis, and the influence of modern technological advancements.
